      *Winter Practice – TUESDAYS 7p-8:10p and THURSDAYS 5:40p-6:50p
      Fall Practices at City Ice Pavilion TUESDAYS 4p-5:20p on 9/11, 9/18, 9/25, 10/2, 10/9, 10/16
      36 practices and 20 games plus Lasker Cup
      COST = $1800.

      *Any child born 1999 or older should contact Director if they are interested in participating.
             CPIH has no program for those ages but neighboring programs do.

      • You can only register one participant at a time. Please submit form then return to register another.
      • Hockey programs fill up quickly, so register quickly!
      • Participants should have their own equipment. Equipment available for seasonal rental. Please consult Director with any questions.
      • Any league participants must be registered with USA Hockey.
      • Lasker Rink & CPIH have a no refund policy.
      • All parents MUST READ and CONFIRM the Parent Creed. Not doing so will not allow you to register your child this season.

      *EARLY REGISTRATION DISCOUNT

      • 10% discount if registration is received between 8/1/12 and 9/4/12
      *FAMILY PLAN
      • Enroll one player at full price.
      • Enroll second player at 10% off that player.
      • Enroll third player at 15% off.

      *Discount is applied to the lower priced registrant each time.
